In partnership with leading health systems, Cadence consistently monitors tens of thousands of patients to improve outcomes, reduce costs, and help patients live longer, healthier lives.</span></p></div><p data-start="141" data-end="647"><span style="color: rgb(0, 0, 0);">Cadence Health is seeking a PRN Registered Nurse responsible for monitoring and triaging patients based on vital signs and clinical alerts, and for supporting patient treatment plans and medication management in collaboration with Cadence Nurse Practitioners and the patient’s physician. PRN staff provide coverage when regularly scheduled team members are unavailable due to vacation, sick leave, or continuing medical education. This role includes coverage across day, night, weekend, and holiday shifts.</span></p>
<p data-start="512" data-end="540"><span style="color: rgb(0, 0, 0);"><strong data-start="512" data-end="538">Licensure Requirements</strong></span></p>
<ul>
<li style="color: rgb(0, 0, 0);" data-start="543" data-end="586"><span style="color: rgb(0, 0, 0);">Active<strong> </strong>Compact<strong> Multi-State RN </strong>license is required.</span></li>
<li style="color: rgb(0, 0, 0);" data-start="543" data-end="586"><span style="color: rgb(0, 0, 0);">An additional RN license in one or more non-compact states.</span></li>
</ul>
<p><span style="color: rgb(0, 0, 0);"><strong>Schedule Requirements:</strong></span></p>
<ul>
<li style="color: rgb(0, 0, 0);"><span style="color: rgb(0, 0, 0);">Minimum of <strong>4 shifts a month</strong> (if needed), with <strong>priority for nights/weekends </strong>if offered.</span></li>
<li style="color: rgb(0, 0, 0);"><span style="color: rgb(0, 0, 0);">Coverage of <strong>2 nights per quarter or 1 weekend per quarter.</strong></span></li>
<li style="color: rgb(0, 0, 0);"><span style="color: rgb(0, 0, 0);">Coverage of <strong>2 holidays per year.</strong></span></li>
</ul>
<p><span style="color: rgb(0, 0, 0);"><strong>What you'll do:<br></strong></span></p>
<ul>
<li style="color: rgb(0, 0, 0);"><span style="color: rgb(0, 0, 0);">Respond to inbound patient clinical questions and escalations by phone (and/or text message).</span></li>
<li style="color: rgb(0, 0, 0);"><span style="color: rgb(0, 0, 0);">Follow standardized triage protocols for incoming calls and escalate appropriately (e.g. Schmitt - Thompson clinical telephonic triage guidelines). This can range from directing the patient to their clinic after-hours call line for non-urgent needs to sending the patient to the ER and/or dialing 911 for emergencies.</span></li>
<li style="color: rgb(0, 0, 0);"><span style="color: rgb(0, 0, 0);">Support clinical documentation for the patients’ Cadence care team (NP, RN, and Patient Success); including documenting patient case notes in the Cadence platform and logging tickets/follow-up tasks in ZenDesk (dedicated ZenDesk training provided).</span></li>
<li style="color: rgb(0, 0, 0);"><span style="color: rgb(0, 0, 0);">Follow up with patients with abnormal readings to gather more information on their clinical status and triage appropriately.</span></li>
<li style="color: rgb(0, 0, 0);"><span style="color: rgb(0, 0, 0);">Monitor patient vitals, symptoms, and labs to identify patients in need of clinical interventions and appropriate escalation.</span></li>
<li style="color: rgb(0, 0, 0);"><span style="color: rgb(0, 0, 0);">Clinical documentation on alert resolution/follow-ups in the Cadence platform.</span></li>
<li style="color: rgb(0, 0, 0);"><span style="color: rgb(0, 0, 0);">Additional administrative support as needed: Follow up with patients who need rescheduling, reach out to patients who have had lapses in taking their vitals, and other administrative work.</span></li>
<li style="color: rgb(0, 0, 0);"><span style="color: rgb(0, 0, 0);">Ensure every healthcare interaction with Cadence is an exceptional experience that prioritizes the well-being of the patient and aligns with the goals of our health system partners.</span></li>
</ul>
<p><span style="color: rgb(0, 0, 0);"><strong>What you need:</strong></span></p>
<ul>
<li style="color: rgb(0, 0, 0);"><span style="color: rgb(0, 0, 0);">An active RN compact multi-state license, at least one additional RN license in a non-compact U.S. state, and a willingness to obtain additional state licensure as Cadence expands into new markets.</span></li>
<li style="color: rgb(0, 0, 0);"><span style="color: rgb(0, 0, 0);">3+ years of experience treating patients with chronic conditions including CHF, Hypertension, and/ or Diabetes patients either in an outpatient or inpatient setting.</span></li>
<li style="color: rgb(0, 0, 0);"><span style="color: rgb(0, 0, 0);">Passion for the patient/customer experience and systematically improving healthcare with digital innovation.</span></li>
<li style="color: rgb(0, 0, 0);"><span style="color: rgb(0, 0, 0);">Independent thinker/operator (comfortable working in a less structured environment).</span></li>

<p><span style="color: rgb(0, 0, 0);">The anticipated compensation range for this role is <strong>$35 to $40 per hour, with an additional $10 per hour for night shifts, </strong>which is based on role scope, level, and location. This position is remote and based in the United States.
