This role is hands‑on, relationship‑driven, and essential to helping patients remain safely and comfortably where they want to be—at home.
What You’ll Do:

- Support household needs such as light housekeeping, laundry, and meal preparation (after completing personal care tasks).
- Use mechanical devices and proper body mechanics to ensure safe mobility assistance.
- Observe and report changes in physical or mental condition, intake/output, and overall patient status.
- Cue patients for medications as directed in the care plan.
- Maintain a clean, safe, and healthy environment following aseptic and safety standards.
- Communicate professionally with patients, families, and the care team.
- Complete timely and accurate documentation.
- Uphold professional boundaries and represent the agency with excellent customer service.
- Travel within the agency’s service area to provide care.
Who You Are:
- A Licensed Nursing Assistant in good standing with the New Hampshire Board of Nursing.
- CPR certified, with a satisfactory background check and motor vehicle record.
- Reliable, compassionate, and committed to delivering exceptional patient‑centered care.
- Comfortable working independently in the home setting while collaborating with a supportive clinical team.
- Preferred: experience in home care and/or caring for patients with dementia, Alzheimer’s disease, COPD, cancer, or post‑surgical needs; ability to perform PT/INR checks.
Why You’ll Love Us:
- Health, dental, prescription, and vision coverage for full‑time & part‑time employees
• Short‑term, long‑term disability, life & pet insurance
• Tuition reimbursement & Nursing Student Loan Paydown Program
• 403(b) retirement savings plans
