Job description

Community Integrated Health Emergency Care Provider opportunity in beautiful Hamilton, Montana!!
Bring your EMS skills to a different kind of call
Bitterroot Health is building out its Community Integrated Health Program, and we are hiring an Advanced EMT or Paramedic who wants to practice differently. Instead of responding to an emergency and handing the patient off, you will follow them home.
In this role you visit patients where they live including houses, ranches, assisted living, and trailers at the end of long gravel roads. You check on the CHF patient who was discharged four days ago and is not sure which of her pills she has already taken. You change a dressing, so a rancher does not drive forty minutes to the clinic for it. You set up a blood pressure cuff that transmits readings to his care team and sit with him until he can use it himself. You open a tablet, get a provider on screen, and be the hands and eyes in the room during that visit.
The work is autonomous, unhurried compared to a rig, and built around the same patients’ week after week. If the part of EMS you like best is assessment, teaching, and relationships, this is the role for you!
What you do

Examine, screen, treat, and coordinate services for patients enrolled in the Community Integrated Health program
Conduct post-discharge follow-up visits: medication reconciliation, wound and dressing care, vital signs, and reassessment
Facilitate telehealth visits in the patient’s home including setting up the encounter, reporting your assessment findings, and troubleshooting the technology
Deploy, teach, and maintain remote patient monitoring equipment
Provide patient education on diabetes, hypertension, CHF, COPD, nutrition, fall risk, and geriatric frailty
Deliver care within your scope of practice, under EMS Services Treatment Guidelines, provider orders, and the CIHC patient care plan
Escalate promptly when a patient’s need exceeds your scope, and stay in regular contact with the EMS Service Medical Director
Coordinate appointments and follow-up with physicians and hospitals
Complete program documentation and the data reporting required under the grant

Scope of practice
You will practice at the full Montana scope of practice as defined by the Montana ECP Scope of Practice Document, including airway management and CPAP, supraglottic airways, IV and IO access, non-medicated IV fluid maintenance, blood glucose monitoring, venous blood sampling, 12-lead ECG acquisition and transmission, splinting, and AEMT-level medications, among them albuterol and ipratropium, epinephrine, naloxone, glucagon and dextrose, ondansetron, parenteral NSAIDs, nitroglycerin, aspirin, acetaminophen, thiamine, and immunizations.
Endorsements expand their scope. If you hold the AEMT Medication Endorsement or AEMT-99 Endorsement, bring them. If you do not, we will support you in earning them. A full scope of practice summary is included in the job description.
Minimum Qualifications:

High school diploma or equivalent
Current Montana ECP license, at the AEMT or Paramedic level, in good standing
Community Integrated Healthcare (CIHC) endorsement at the AEMT or Paramedic level, or willingness to obtain one within six months of hire, we will help you get there
Current BLS/CPR certification
Valid Montana driver’s license and an excellent driving record
Sound judgment, comfort working independently, and the self-awareness to know where your scope ends

Preferred Additional Qualifications

Two or more years of field EMS experience
AEMT Medication Endorsement, AEMT-99 Endorsement, or Paramedic
Experience with chronic disease management, home visits, or care coordination

Schedule: Full-time, 40 hours/weekBenefits: This position is eligible for the full benefit package

This is a grant funded term position. Employment is expected to run through 9/15/2027, with continuation contingent on renewed RHTP award funding and program performance. Bitterroot Health intends to pursue continuation funding and, where the program demonstrates results, to sustain the role beyond the grant period. We would rather tell you that plainly up front than bury it in an offer letter.
