Responsibilities

Evaluate and develop individualized treatment plans for patients.
Deliver high-quality therapy services in accordance with professional standards and physician orders.
Collaborate with interdisciplinary team members to maximize patient outcomes.
Accurately document evaluations, treatments, progress notes, and discharge summaries.
Educate patients, families, and caregivers on treatment plans and goals.
Maintain compliance with regulatory, ethical, and organizational requirements.

Qualifications
Physical Therapist (PT)

Current state Physical Therapist license in good standing.
Graduate of an accredited Physical Therapy program.
CPR certification preferred.

Occupational Therapist (OT)

Current state Occupational Therapist license in good standing.
Graduate of an accredited Occupational Therapy program.
CPR certification preferred.
