Help patients get covered and stay connected to care.

Gracelight Community Health is seeking an Enrollment Specialist to help individuals and families obtain and maintain health coverage. Based at a health center, you’ll guide patients through applications and renewals, explain their benefits, and help resolve problems that could interrupt their care.

We’re looking for someone who is patient, organized, and comfortable explaining complicated information in everyday language. Your follow-through will help patients navigate coverage decisions and meet important deadlines.

What you’ll do

• Help patients apply for and renew coverage through Medi-Cal, Covered California, Medicare, and other available programs.

• Contact existing patients approaching renewal or redetermination deadlines and those becoming eligible for Medicare.

• Meet with patients in person and by phone to understand their needs, gather documents, and complete accurate applications.

• Track applications, follow up on outstanding items, and help resolve coverage problems and appeals when appropriate.

• Explain plan benefits, how to use coverage, and what patients need to do to keep it active.

• Work with care teams and front-office staff to connect uninsured patients with enrollment assistance.

• Help patients register for MyChart and connect them with community resources when needed.

• Maintain accurate patient records, application tracking, and monthly reports.

• Stay current on program changes through ongoing training and maintain Certified Enrollment Counselor certification.

• Occasionally participate in community health fairs and enrollment events.

Qualifications

What you’ll bring

• Lived experience in the communities Gracelight serves and/or at least one year of direct service experience in healthcare, enrollment, or a community-based setting.

• Bilingual English/Spanish skills.

• The ability to obtain and maintain Covered California Certified Enrollment Counselor (CEC) certification.

• Strong communication skills and the ability to support patients from diverse backgrounds with respect and sensitivity.

• Attention to detail and the ability to manage multiple applications, deadlines, and follow-up tasks.

• Proficiency with Microsoft Office applications and comfort using online enrollment systems.

Work location and schedule

This is an onsite position based at a Gracelight health center, with flexibility to support other locations and occasional community events. The schedule may include evenings and weekends.
