Licensed Practical Nurse

We are seeking a compassionate and dependable **Licensed Practical Nurse (LPN)** to join our Adult Day Health team. The LPN will provide direct nursing care and clinical support to participants, helping promote their health, safety, independence, and overall quality of life. Working closely with the RN and interdisciplinary Care Team, the LPN will implement individualized care plans, administer medications and treatments, monitor changes in participants' health and functioning, and provide appropriate clinical support.
