RN Assessment Nurse

As an RN Assessment nurse at Christopher's Bridge Home Care you will be responsible for the following: 

1. Schedule and complete an initial Nurse Assessment for new clients.

2. Schedule and complete routine Supervisory client visits, every three months (90 days).

3. Review and co-sign all documentation done by the caregivers on a weekly basis.

4. Review routinely office emails for any client concerns that need nursing follow-up such as caregiver updates On changes needed on the client's care plans, client/family requests or Other client concerns.

5. Review "Alerts" on the Axis Client chart portal for any needs for nursing follow-up.

6. Participate in bi-weekly telephone or in-person Client Care Coordination meeting with RN Care Coordinator. 

7. Coordinate client care by telephone Or in person (usually when completing client's Supervisory Visits or when there are changes in client care ) with client's POA's/family, Hospice, Home Health and/or facility(ALF and NH) staff and make any necessary updates on client's Care Plans.

8. Schedule and train caregivers on Proxy skills as needed at client's home Or at the office if equipment is available.

9. Attend routine company meetings as scheduled.

10. Additional tasks as directed/needed by company's Executive Director and/or office administration.
