Community Health Worker

Monroe Health Center is seeking a Full-time Community Health Worker.  The main work location for this position will be the Forest Hill Health Center in Forest Hill, WV.  

The Community Health Worker (CHW) supports the mission of our Federally Qualified Health Center (FQHC) by helping patients access healthcare services, connect with community resources, and overcome barriers to care. The CHW serves as a link between patients, healthcare providers, and community organizations to promote improved health outcomes and overall well-being.

Essential Duties and Responsibilities:

• Assist patients with accessing medical, behavioral health, and preventive care services.
• Identify barriers to care, including transportation, housing, food insecurity, insurance, and other social needs.
• Connect patients with appropriate community resources and support services.
• Provide health education and encourage preventive care and healthy lifestyle choices.
• Conduct patient outreach, follow-up calls, and referrals as needed.
• Support care coordination and help patients navigate the healthcare system.
• Document patient interactions, referrals, and services provided in the electronic health record or other designated systems.
• Participate in community outreach events, health fairs, and wellness programs.
• Maintain patient confidentiality and comply with HIPAA requirements.
• Perform other duties as assigned.

Qualifications:

• High school diploma or GED required.
• Community Health Worker certification preferred; willingness to obtain certification may be considered.
• Experience in community outreach, healthcare, social services, or patient navigation preferred.
• Strong communication, interpersonal, and organizational skills.
• Ability to work effectively with individuals from diverse backgrounds.
• Knowledge of community resources and social determinants of health is a plus.
• Basic computer skills and the ability to maintain accurate documentation.
