MDS/Care Plan Coordinator

Ridgecrest Village is seeking an experienced MDS/Care Plan Coordinator to join our team. In this role, you'll help ensure high-quality resident care through accurate assessments, individualized care planning, regulatory compliance, and clinical reimbursement support.

Responsibilities

• Complete and submit accurate MDS assessments
• Coordinate and maintain individualized resident care plans
• Collaborate with the interdisciplinary team to support resident outcomes
• Ensure compliance with CMS, Medicare, Medicaid, and state regulations
• Assist with quality improvement initiatives and survey readiness
• Provide education and support related to MDS and care planning processes

Qualifications

Qualifications

• Current Iowa RN or LPN license
• Minimum 2 years of long-term care experience
• Strong assessment, documentation, and communication skills
• MDS experience preferred
• PointClickCare experience preferred
