The Coordinator II – Youth and Family Support Services

Pay: $26.00-$27.00/hour

Help Young Parents Build Stronger Futures

Do you believe young parents deserve support that recognizes their strengths, respects their choices, and meets them where they are?

As a Case Manager II in our Young Parent Support (YPS) Program, you’ll work directly with expectant and parenting young people ages 14–24, helping them create safe, stable, and nurturing lives for themselves and their children. You’ll build meaningful relationships with young parents in their homes and communities while helping them navigate services, reach their goals, and build the skills and confidence they need to thrive.

This is a hands-on, relationship-centered role where your ability to listen, advocate, encourage, and connect people to the right resources can make a lasting difference.

Due to the nature of behavioral health services for children and families, exempt employees are expected to maintain a work schedule during the hours of operations that meets client needs, program needs, and job performance expectations. Support and Stabilization services typically operate from 8:30 am to 8pm Monday through Friday. In some situations, needs of families may require weekend availability. Your schedule will include afternoon and evening hours to ensure that you can serve the needs of your caseload and/or supervisory responsibility, as well as one week per year that you may be on call. Work schedules may vary based on service delivery requirements, family availability, and organizational needs.

Benefits for Full-time employees:

  • Health Insurance: Offered through Blue Cross Blue Shield, with generous company contribution.

  • Enhanced Retirement Plan: 25% - 33% Employer match based on tenure.

  • Discounted Tuition with College & University Partnerships!

  • Tuition Assistance: Reimbursed or prepaid college coursework!

  • Home Mortgage Initiative: Favorable down payment (3-5%), reduced closing costs co-paid by Seven Hills!

Work-Life Balance:

  • Vacation Cash-Out Option

  • 3 Paid Personal Days

  • 11 Paid Holidays

What You’ll Do:

  • Provide strength-based, trauma-informed case management and support to pregnant and parenting youth ages 14–24.

  • Build trusting relationships with young parents through regular home and community visits.

  • Work alongside participants to identify their goals and develop individualized service plans that reflect their priorities and strengths.

  • Help young parents access resources such as childcare, housing, education, employment, health care, WIC, Early Intervention, and behavioral health services.

  • Provide parenting education, coaching, and practical support using evidence-informed approaches and curricula.

  • Help young parents develop parenting, life, and self-sufficiency skills that support long-term stability and independence.

  • Coordinate care with DCF workers, schools, healthcare providers, childcare agencies, and other community partners.

  • Advocate for participants while supporting their voice, choice, independence, and ability to make decisions about their own lives.

  • Track goals and progress through regular visits, communication, and documentation.

  • Maintain accurate and timely case records in accordance with agency and funder requirements.
