Family Nurse Practitioner

Sandhills Medical Foundation, Inc. is seeking a dedicated and compassionate Family Nurse Practitioner (FNP) in Sumter, SC. As a Federally Qualified Health Center (FQHC) serving the community since 1977, our mission is to deliver quality, comprehensive, and cost-effective healthcare to responsive community needs. In this role, you will function as an essential member of a Patient-Centered Medical Home, providing primary care to patients across the lifespan.

Key Responsibilities

• Diagnose and treat acute illnesses, injuries, and manage chronic conditions.
• Provide comprehensive primary healthcare and wellness exams within established guidelines.
• Order and interpret laboratory studies and diagnostic testing.
• Prescribe therapeutic treatment plans and medications.
• Coordinate care with specialists, testing facilities, and community health teams.
• Maintain accurate, HIPAA-compliant patient charts within the Electronic Medical Record (EMR) system.

Qualifications

Qualifications

• Education: Master's or Doctoral degree in Nursing (MSN/DNP) from an accredited program.
• Licensure: Board certified Family Nurse Practitioner (FNP) with an active, unrestricted Nurse Practitioner license.
• Experience: 1–2 years of experience in family practice, internal medicine, or community health environments is preferred.
• Skills: Proficient with EMR charting, excellent customer service, and strong team-oriented clinical skills.
