Clinical Nurse Liaison (RN) Responsibilities:

• Maintains regular contact with acute care facility referral partners, including discharge planners, case managers, physicians, hospitals, nursing staff, and other appropriate clinical personnel.
• Receives, tracks, and follows through on hospice referrals from acute care facilities.
• Evaluates patients and coordinates or completes the admission process, as assigned
• Explains hospice services and care options to patients, families and completes required admission documentation. 
• Coordinates with hospital case managers and hospice teams to support timely admission, safe discharge and transition to appropriate setting. 

Clinical Nurse Liaison (RN) Qualifications:

• Registered nurse RN with current licensure to practice professional nursing in Pennsylvania.
• Graduate of National League for Nursing accredited school of nursing.
• Minimum of two years of experience; experience in the area of hospice nursing.
• Self-directed and able to work with minimum supervision.
• Demonstrated ability to prioritize tasks and handle pressure and delicate situations.
• A state driver’s license in good standing, and maintain an automobile in good working order.
• High proficiency with computers, Microsoft Office, cellular technology, and EMR required.
• Able to perform the physical tasks that are needed in this role.
