  • Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards.
  • Collaborate with patients, caregivers, physicians, and the interdisciplinary team to optimize mobility, functional outcomes, and fall prevention.
  • Educate patients and caregivers on therapeutic exercises, gait training, transfer techniques, fall prevention, assistive devices, and individualized Home Exercise Programs (HEPs) to promote safe mobility and recovery.
  • Reassess patient progress, modify treatment plans as appropriate, and communicate changes to the care team.

💼 What We're Looking For:

  • Degree from an accredited Physical Therapy (PT) or Physical Therapist Assistant (PTA) program.
  • Current Michigan Physical Therapist/Physical Therapist Assistant license in good standing.
  • Home health experience preferred (minimum one year).
  • Strong clinical judgment, critical thinking, and problem-solving skills.
  • Excellent communication, interpersonal, and patient education skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Reliable transportation, a valid driver's license, and current auto insurance.

🎁Why You'll Love Working Here

  • Competitive pay with weekly payroll
  • Flexible scheduling for better work-life balance
  • Medical benefits for employees working 30+ hours/week
  • 401K Retirement Plan
  • Paid Time Off (PTO) eligibility
  • Paid orientation and onboarding
  • Employee recognition and appreciation programs
  • Supportive, team-focused culture with meaningful work
